Thunder Bay’s health unit is making it easier to consent for school vaccine clinics.
They have launched a new secure online form to make it easier for parents and guardians across the district.
The new online form is quick, can be viewed on a mobile phone, and is available in English and French.
“The online consent form was developed in response to feedback from parents and school staff and we are excited to be able to offer this option,” said TBDHU’s Manager of Vaccine Preventable Disease, Laura Dennis. “We want to make providing consent easy for busy parents and guardians.”
Grade Seven students in the Thunder Bay area have already received their information packages.
Students attending Grade Seven in District communities will receive their information packages through their schools this fall.
Students attending high schools in the District will receive their information packages from the health unit by mail in the fall.
The health unit is encouraging parents and guardians to provide feedback on the online form so that TBDHU can continue to improve the process.
Forms will be available on their website on September 23.
Parents and guardians who have not received an information package can find all documents, including the online consent form, and a school clinic schedule at TBDHU’s School Immunization
page here.
